What to Bring?
There are a few important items you will need to bring with you to your appointment. They are as follows:
- Your insurance card and photo ID.
- A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency. This will allow your provider to make sure you are on the right combination of medications.
- A list of any herbal supplements or vitamins you are taking, since any of these can affect your body and its reaction to other medications we may prescribe for you.
- Pertinent information about your medical and surgical history.
- Any recent x-rays and/or MRI reports and, if possible, please have your previous medical records (ONLY the most recent) sent to our office prior to your visit. This will aid the physician with your treatment.
- Faxes from other offices are limited to 10 pages and it is not necessary to have the complete chart forwarded to our office. (These records are NOT needed and will be returned to the patient.) If there is anything else required, we will inform you.
Prior to your first appointment....
Prior to your first appointment, you will need to complete the new patient forms found in the drop down box under the PATIENT tab. Please bring the completed forms with you to your first office visit. There are 2 other forms which are proprietary in nature and you will need to fill them out when you arrive for your appointment. If you are unable to access or print the forms, we ask that you arrive 30 minutes before your appointment time in order to complete the new patient registration forms. If you have the above mentioned forms filled out, please arrive 20 minutes prior to your appointment time.
